The CC holder holder does not have to be the same person as the account holder.
It's the same with any service. The service provider is not required to perform due diligence on the validity of every payment is receives for it's services. That responsibility falls to the credit card company.
Ok then. how about this....
Its not really just me logged into two accounts (one mine, one my friends). My friend is really sitting right next to me sharing computer time. when my char is active, i am at the keyboard. When the other active, my (make believe) friend is at the keyboard. Two accounts, owned by seperate people, running on the same computer at the same time. Yes, rediculous. BUT, possible. Is there any way to disprove this if confronted? I dont think so. So how can this be enforced? I'm sure there are holes in this line of thinking. Let me know what you come up with.
Edit: I have at no time dual logged, by either logging two chars on one account or two chars on seperate accounts. I am merely playing devils advocate and please oh please dont take my fun away.
